The riskiest moment in any recovery is the day of discharge. New medications, new instructions, new equipment — and often, no one at home to translate it all. Our Hospital to Home service is built specifically for that handoff: we walk in with you and stay until things feel steady.
What's included
- Discharge-day pickup and transport coordination
- Setting up the home for safety and accessibility
- Medication organisation and first-dose support
- First-week care plan with daily visits if needed
- Follow-up scheduling with your physician
Who it's for
- People being discharged with complex medication or wound-care needs
- Patients without family nearby on discharge day
- Hospital social workers and discharge planners coordinating safe returns
Our approach
We can be reached the day you’re admitted — earlier is better. Our team meets the discharge team, brings the right equipment home, and provides the first weeks of intensive support so the recovery doesn’t stall.
