A hospital stay is rarely just a stay — it bookends weeks of preparation and recovery. We help with both ends. Whether you’re organising things before an admission or settling back in afterwards, our team is the steady presence that keeps the small details from becoming big problems.
What's included
- Pre-admission preparation and home set-up
- Discharge coordination and transport home
- Medication reconciliation after discharge
- Wound monitoring and follow-up appointment reminders
- Mobility, bathing, and meal support during recovery
Who it's for
- Anyone preparing for a planned surgery or hospital admission
- Patients being discharged with new medications or care instructions
- Family members coordinating recovery from a distance
Our approach
We start with a free in-home conversation about what the recovery period will look like — what you’ll need on day one, on day seven, and a month in. Our nurses and caregivers then build a tapered care plan that matches your recovery curve and steps back as you regain independence.
